**Job Details:**

We are seeking a responsible, patient, and qualified Licensed Practical Nurse to assist with reviewing patient charts. The role involves ensuring the accuracy of patient records and documents to reflect each patient's conditions and treatment requirements.

**Key Responsibilities:**

  • Review patient records for accuracy and completeness
  • Submit Review Visit Logs to supervisor for review and approval
  • Adhere to operational guidelines and protocols
  • Respond to correspondences and inquiries in a timely manner
  • Submit clinical corrections and updates as necessary
  • Attend meetings and training sessions as scheduled
  • Communicate with Operations for IT issues and technical support
  • Cross-train in care management and patient care coordination
  • Utilize electronic medical records (EMRs) and other healthcare systems

**Requirements:**

  • LPN certificate/degree from an accredited institution
  • Ability to work full-time in a fast-paced healthcare environment
  • Empathetic and active listening skills
  • Detailed-oriented and organized
  • Effective time management and prioritization skills
  • Strong communication and interpersonal skills
  • Experience with EMR systems and healthcare technology
