About the Role

We are seeking a skilled Registered Nurse to join our team at Illumia, where you will play a vital role in providing exceptional care to patients and their families. As an Afterhours Palliative Care RN, you will be responsible for identifying and coordinating patient/family needs, providing preventive and supportive care, and utilizing assessment, intervention, and teaching skills to maximize patient comfort and enhance the quality of life.

Key Responsibilities
  • Triage patient needs over the phone and respond to crisis calls from patients and families after hours
  • Make initial nursing evaluations to determine eligibility for palliative services within 48 hours of referral
  • Identify patient/family physical, psychosocial, emotional, and environmental needs and reassess as needed
  • Collaborate with patients, families, attending physicians, and other members of the IDG to provide patient and family care
  • Document patient problems, goals, interventions, and response to palliative care
  • Communicate effectively with the AIM team, clinical supervisor, and patient's PCP regarding patient status and needs
Requirements
  • Associates/Bachelor's degree in Nursing
  • 2 years of prior skilled Registered Nurse experience
  • Demonstrated ability to provide quality patient care
  • Nursing experience in palliative care, oncology, hospice, or geriatric care preferred
