About the Role

We are seeking a compassionate and skilled Registered Nurse to join our team as an Afterhours AIM (Advanced Illness Management) Program RN. This is a full-time position with a 7on/7off schedule, reporting directly to the Director of AIM.

Key Responsibilities
  • Identify and coordinate patient/family needs, providing preventive and supportive care to palliative patients and families.
  • Utilize assessment, intervention, and teaching skills to maximize patient comfort and enhance quality of life.
  • Assume afterhours coverage for designated teams and function as an interdisciplinary advanced illness management and support, including telephonic, video, and in-person interventions.
Requirements
  • Current RN license to practice in the state.
  • CPR Certification.
  • 2 years of prior skilled Registered Nurse experience.
  • Demonstrated ability to provide quality patient care.
  • Nursing experience in palliative care, oncology, hospice, or geriatric care preferred.
