We are seeking a highly skilled Nurse Practitioner to join our Senior Community Care team at UnitedHealth Group. As a key member of our interdisciplinary team, you will provide comprehensive care to patients in their homes, nursing homes, and assisted living facilities.
Key Responsibilities- Conduct thorough assessments of patients' medical needs and develop personalized care plans
- Manage patients with acute medical situations, chronic conditions, and short-term rehabilitation needs
- Collaborate with the care team to ensure seamless care coordination and communication
- Provide advanced illness and end-of-life discussions, as well as counseling and education to patients and caregivers
- Master's degree in Nursing or Doctor of Nursing Practice from an accredited program
- Active and unrestricted license to practice as a Nurse Practitioner in the state where you reside
- Board certification through a national board, such as the American Academy of Nurse Practitioners or the American Nurses Credentialing Center
- Current DEA licensure and prescriptive authority, or ability to obtain post-hire
- Ability to gain a collaborative practice agreement, if applicable in your state
- Access to reliable transportation
- 1+ years of clinical experience in a long-term care setting
- Experience in meeting the medical needs of patients with complex behavioral, social, and functional needs
- Understanding of Geriatrics and Chronic Illness
- Understanding of Advanced Illness and end-of-life discussions
