About the Role

LHC Group is seeking a skilled Physical Therapist to join our team in providing high-quality, patient-centered care in the comfort of patients' homes.

Key Responsibilities
  • Evaluate patients' functional status and physical therapy needs, collaborating with physicians to develop personalized therapy plans.
  • Provide services within the scope of practice, adhering to state laws and guidelines, and coordinating with healthcare teams to ensure comprehensive care.
  • Monitor patients' responses to treatment and report any changes to supervising nurses and physicians.
  • Develop and implement treatment plans to relieve pain, restore function, and maintain maximum functioning, utilizing accepted standards of practice.
  • Communicate with healthcare teams, patients, and caregivers regarding treatment plans, functional limitations, and progress toward goals.
  • Complete progress notes for each visit and submit them according to LHC Group policy.
Requirements
  • Current Physical Therapy licensure in the state of Florida.
  • Current CPR certification.
  • A valid driver's license and vehicle insurance, with access to a dependable vehicle.
