Job Overview

We are seeking a skilled Hospice Registered Nurse/RN Case Manager to join our team. This role will involve providing direct patient care services, as well as coordinating with interdisciplinary teams to ensure high-quality care.

Responsibilities

  • Provide direct patient care services to hospice patients and supportive care to their families
  • Act as primary liaison between physician, patient/family and hospice team
  • Evaluate patient and family needs and assist in the preparation and execution of the plan of care for each patient
  • Maintain all assigned patient clinical records
  • Participate in company quality assessment and performance improvement programs

Requirements

  • Bachelor's degree in nursing from an accredited program
  • RN license in the state of employment (or eligibility to obtain)
  • 1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology
  • Current driver's license and automobile insurance
  • Current CPR certification
