Job Summary

Oaks Integrated Care is seeking a highly skilled Access Coordinator to join our team. As an Access Coordinator, you will play a critical role in facilitating linkage into our clinical mental health and substance use treatment programs.

Key Responsibilities
  • Collaborate with interdepartmental clinical teams and referral sources to ensure seamless coordination of care.
  • Navigate the continuum of care with individuals seeking treatment, considering their holistic needs and ensuring linkage with all applicable services.
  • Process requests for school clearances and schedule assessments, utilizing strong organizational and communication skills.
  • Triage incoming referrals and offer appropriate levels of care, demonstrating expertise in mental health diagnostic criteria.
  • Assist the Access Coordinator with the collection and maintenance of data systems, ensuring accuracy and efficiency.
  • Manage charting in agency electronic clinical records, adhering to standards set forth by the Agency and funding sources.
  • Proficiency in Microsoft applications and general computer operations is required.
Requirements
  • Bachelor's degree in a Human Services-related field.
  • Valid Driver's License.
