About the Role

We are seeking a highly skilled Registered Nurse to join our team as an Afterhours Advanced Illness Management Registered Nurse. As a key member of our interdisciplinary team, you will be responsible for providing high-quality patient care and support to individuals with advanced illnesses.

Key Responsibilities
  • Provide preventive and supportive care to palliative patients and families
  • Assess patient needs and develop individualized care plans
  • Collaborate with healthcare teams to ensure seamless care coordination
  • Respond to crisis calls and provide urgent support to patients and families
  • Document patient information and progress in a timely and accurate manner
Requirements
  • Current RN license to practice in the state
  • 2 years of prior skilled Registered Nurse experience
  • Demonstrated ability to provide quality patient care
  • Nursing experience in palliative care, oncology, hospice, or geriatric care preferred
