Job Summary

We are seeking a highly skilled Palliative Care Nurse Practitioner to join our team at Trinity Health. As a key member of our interdisciplinary team, you will provide comprehensive care to patients and families facing serious illnesses.

Key Responsibilities
  • Conduct thorough medical and psychosocial evaluations of patients and families to develop individualized care plans.
  • Mentor physicians and nurses to support the primary care team and promote best practices in palliative care.
  • Implement palliative care protocols in multiple ICUs and develop screening processes for ICU palliative consults.
  • Lead discussions on care goals with ICU teams and independently follow up on patient consultations and symptom management.
  • Participate in ICU rounds and conduct consults throughout the hospital, including the ED.
Education and Experience
  • Master's Degree in Advanced Practice Nursing required.
  • At least 1 year of experience in inpatient Palliative Medicine required.
  • National advanced practice certification in adult, geriatrics, or family practice preferred.
