Job Summary:

Liberty Health is seeking an experienced Hospice Registered Nurse (RN) to join our team. As a Hospice Registered Nurse, you will play a vital role in providing high-quality patient care and support to individuals and their families facing terminal illness.

Key Responsibilities:

  • Apply a working knowledge of nursing theories and concepts to deliver comprehensive care to patients.
  • Coordinate physical care of the patient with family, patient, physician, and Hospice team to ensure seamless care delivery.
  • Communicate effectively with patients, families, and healthcare professionals at all levels to ensure optimal patient outcomes.
  • Participate in Interdisciplinary Team (IDT) meetings and quality reviews to ensure high-quality patient care and adherence to reimbursement guidelines.

Requirements:

  • Current RN licensure in the state of practice.
  • Successful completion of a nursing education from an approved school of nursing.
  • At least two years of clinical nursing experience, with a minimum of one year of hospice nursing experience.
  • Working knowledge of the principles and practice of nursing according to nursing practice act, agency policies and procedures, and reimbursement guidelines.
  • 12 hours of continuing education annually.
  • CPR certification.
  • Ability to accept travel assignments and work flexible hours, including weekends, evenings, and holidays.
