Key Responsibilities:

  • Promote and Achieve Our Mission: Align with our organization's mission and vision to connect with and care for our community, ensuring everyone enjoys health and wellness.
  • Assess Patient Needs: Evaluate patients' clinical, social, functional, and continuing care needs to identify their priorities and desired outcomes for their treatment.
  • Develop Care Plans: Collaborate with patients, their care team, and related partners to develop care plans, including immediate, short-term, and long-term goals reflecting patients' priorities and timeframes to achieve desired outcomes.
  • Transition of Care: Assist in the transition of care through patient outreach, communication with relevant service providers following discharge from other care settings to ensure a smooth transition to outpatient care, with the goal of decreasing readmissions.
  • Effective Communication: Communicate with patients and families in an effective, patient-centered manner; coordinate with other team members to provide exceptional patient service.
  • Positive Working Relationships: Maintain positive working relationships and demonstrate exceptional customer service skills, working cooperatively, addressing conflict, and communicating effectively with all providers, team members, patients, and their families.
  • Patient Access: Facilitate patient access to appropriate medical and specialty providers as well as other care coordination team support.
  • Chronic Care Management: Provide Chronic Care Management services as appropriate to eligible patients.
  • Effective Tracking: Ensure effective tracking of test results, medication management, and adherence to follow-up appointments.
  • Patient Self-Management: Support patient self-management of disease and behavior modification interventions.
  • Proactive Outreach: Proactively outreach to selected patients, perform home visits as necessary for assessment and engagement.
  • Quality Improvement: Participate in quality improvement activities, collecting and analyzing data.

Requirements:

  • Experience with Care Planning: Proven experience with care planning and coordination.
  • Nursing Experience: Two (2) years of nursing experience, preferably in case management.
  • Excellent Communication Skills: Excellent communication and interpersonal skills.
  • Computer Skills: Proficiency with Microsoft Office and computer skills.
  • Licensure: Current LPN License, with RN License preferred.
