Job Summary:

We are seeking a highly skilled and compassionate Registered Nurse to join our team as a Case Manager. As a key member of our interdisciplinary team, you will be responsible for providing complex nursing care to individuals with intellectual and developmental disabilities who are medically fragile or have behavioral problems.

Key Responsibilities:
  • Conduct comprehensive physical assessments and develop individualized care plans to ensure optimal health outcomes.
  • Collaborate with the interdisciplinary team to coordinate care and services, including medical appointments and treatment.
  • Provide education and support to individuals, their families, and staff members to promote health and wellness.
  • Participate in quality assurance reviews and contribute to the development of nursing policies and procedures.
  • Maintain accurate and up-to-date records, including individual plans of care and comprehensive quarterly and annual assessments.
Requirements:
  • Current RN licensure in the state of Texas.
  • Minimum of 2 years of experience in case management or a related field.
  • Strong knowledge of nursing techniques, standards, and procedures, as well as health care laws and regulations.
  • Excellent communication and interpersonal skills, with the ability to work effectively with individuals, families, and staff members.
  • Ability to work independently and as part of a team, with a strong commitment to providing high-quality care and services.
