  • Providing comprehensive primary medical care to assigned patients, supporting their Medicare Advantage Care Plan.
  • Developing patient-centered care plans based on their values and needs.
  • Evaluating prognoses and structuring care plans, including advanced care planning.
  • Leading interdisciplinary team meetings to ensure effective execution of patient care plans.

Qualifications:

  • MD/DO degree, Board Certified/Eligible in Family, Internal, or Geriatric Medicine.
