Job Summary

Therapeutic Alternatives is seeking a highly skilled and experienced Geriatric Adult Specialty Team Masters Level or Licensed Clinician to join our team. As a key member of our team, you will be responsible for providing education and training to supervisors and direct care staff of assisted living facilities, family care homes, skilled nursing homes, community agencies, and other invested community agencies.

Key Responsibilities
  • Educate and Train long-term care facility staff through in-service training classes, ensuring they have the necessary skills and knowledge to provide high-quality care to geriatric patients.
  • Consult and Collaborate with Long Term Care (LTC) facilities on behavior management, crisis planning, and other appropriate intervention techniques, providing expert guidance and support.
  • Coordinate Care with psychiatrists and other mental health providers, educating LTC facilities and families within the community on how to effectively coordinate care.
  • Provide Case Consultations as requested by LTC facility staff, offering expert advice and guidance to ensure the best possible outcomes for patients.
  • Network and Build Relationships with LTC facilities, community agencies, and other relevant stakeholders, fostering strong partnerships and collaborations to advance the goals of the Geriatric Adult Specialty Team.
  • Document Services Provided, maintaining accurate records of all services delivered to ensure compliance with regulatory requirements and to support billing and reimbursement.
  • Ensure Compliance with regulatory requirements, as appropriate within the scope of the program, to ensure the highest standards of care and service delivery.
  • Assist with Referrals to community-based services as needed, providing support and guidance to LTC facilities and families to access necessary resources.
  • Provide Resource Information to LTC staff relevant to the geriatric population, ensuring they have access to the latest research, best practices, and resources to inform their care.
  • Consult and Collaborate with Other Healthcare Providers as appropriate, working together to ensure seamless care coordination and delivery.
  • Maintain Certifications and Training, ensuring that all necessary certifications and training are up-to-date and compliant with regulatory requirements.
  • Travel and Market Program Services, visiting LTC facilities and community agencies to provide education and training, and to market program services to potential partners and stakeholders.
  • Attend Meetings and Perform Other Duties as required by circumstance, ensuring that the goals and objectives of the Geriatric Adult Specialty Team are met.
Qualifications
  • A Master's prepared individual or Licensed clinician with a degree in a human service area.
  • One year of experience working with the geriatric population, as required by state regulations.
  • One year of experience working with individuals within the mental health and geriatric field.
  • No substantiated findings on the NC Personnel Registry or Health Care Program.
  • A valid NC Driver's License and a clean SBI/DMV Report, with at least 8 years of driving experience.
  • The ability to lift, push, and pull a minimum of 25 pounds, and to sit for periods of time, walk for up to several hours at a time with breaks.
  • The ability to squat and climb at times, and to operate a vehicle in varying weather conditions.
