We are seeking a skilled Case Manager to join our team at House of Mercy Inc. as a Housing Stabilization Specialist. The successful candidate will be responsible for providing case management and service coordination to assist clients in reaching optimal health and maintaining stable housing.
Key Responsibilities- Conduct assessments and develop individualized case plans to address clients' needs and goals.
- Link clients with community resources and provide support to help them achieve economic and social/emotional self-sufficiency.
- Conduct home visits and maintain an average caseload of 13 clients in temporary, transitional, and permanent housing.
- Provide crisis intervention and identify service needs and barriers.
- Maintain compliance with workflows, policies, procedures, and other activities to support the House of Mercy's model and accreditation.
- Licensed Clinical Social Worker in the State of North Carolina.
- Masters degree in social work from an accredited college or university.
- A minimum of 2 years of case management experience with chronically homeless adults, severe & persistent mental illness, and/or substance use disorder.
- Valid driver's license and reliable transportation.
- Ability to read and interpret documents such as leases, medical records, background checks, court documents, and written documents.
- Ability to write reports and develop presentations.
- HUD CPD Part 5 Income Calculation Certificate within 30 days.
- Experience using HMIS or obtain HMIS Certification within the first 30 days.
- SOAR certification or willingness to obtain in the first 90 days.
