Job Title: Home Health RN

We are seeking a compassionate and skilled Registered Nurse to join our team of healthcare professionals in providing high-quality care to patients in their homes.

About the Role:

As a Home Health RN with Caretenders, a part of LHC Group, you will have the opportunity to make a meaningful difference in the lives of our patients and their families. You will be responsible for providing and directing provisions of nursing care to patients in their homes, as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies.

Key Responsibilities:
  • Conduct initial and comprehensive nursing evaluations to determine primary focus of care and develop the plan of care within state-specific guidelines.
  • Regularly evaluate and re-evaluate patient's nursing needs, ensuring timely and accurate documentation.
  • Initiate, develop, implement, and make necessary revisions to the plan of care in collaboration with the physician and other healthcare professionals involved in care.
  • Provide clinical services within the scope of practice, as defined by state laws governing the practice of nursing, in accordance with the plan of care and in coordination with other members of the healthcare team.
Requirements:
  • Current RN licensure in the state of practice.
  • Current CPR certification required.
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
