CareCoach Connect Nurse Practitioner or Physician Assistant

We are seeking a skilled Nurse Practitioner or Physician Assistant to join our CareCoach Connect team in Tyler, Lufkin, Carthage or Henderson, TX. As a member of our team, you will provide high-quality patient care in a collaborative environment that values diversity and inclusion.
Key Responsibilities:- Provide comprehensive care to patients in their homes and virtually, consistent with your licensure and/or certification.
- Diagnose and treat acute and chronic conditions, including diabetes, hypertension, kidney disease, and other chronic medical conditions.
- Order and interpret diagnostic lab work, x-rays, and EKGs.
- Prescribe pharmacological and non-pharmacological interventions for both acute and chronic conditions.
- Perform procedures based on the patient's condition, such as placing IVs, wound debridement, joint injections, and vaccinations.
- Collaborate with lead M.D./D.O, specialists, and other healthcare team members to develop a plan of care.
- Determine the effectiveness of the treatment plan, evaluate patient response, and revise the plan accordingly.
- Communicate effectively with patients and families to ensure understanding of diagnosis, prognosis, and treatment plans.
- Manage census of longitudinal care patients and work with the team to oversee their care.
- Transition patients to hospice or back to usual care when clinically necessary.
- Perform transition of care evaluations and acute visits for patients when clinically necessary.
- Prepare and maintain accurate patient records and documents.
- Provide education to patients and families on chronic disease management, symptom management, and quality of life expectations.
- Participate in on-call responsibilities as required.
