Job Summary:The Clinical Care Coordinator will play a vital role in facilitating care management and providing navigation services to patients. This individual will prioritize patient services, communicate with referral sources, primary care providers, patients, clinicians, and clinical leaders to ensure seamless care coordination.
Key Responsibilities:- Develop and implement care plans in collaboration with patients, families, and healthcare teams.
- Provide education and support to patients and families to promote self-management and independence.
- Communicate effectively with healthcare providers, payers, and community resources to ensure timely and efficient care delivery.
- Monitor patient progress, identify potential barriers, and develop strategies to overcome them.
- Collaborate with interdisciplinary teams to ensure comprehensive care and address complex patient needs.
- Associate's Degree in Nursing required; Bachelor's Degree in Nursing preferred.
- Current RN licensure in Massachusetts.
- Two years of community health experience in a Medicare-certified agency.
- Demonstrated clinical competency and knowledge of home care principles and practices.
- Excellent interpersonal and communication skills.
- Ability to exercise independent judgment and maintain confidentiality.
- Demonstrated familiarity with DPH regulatory standards.
- Coaching/team building skills and ability to handle conflict.
- Flexibility in thought process and time availability.
