As a RN Care Coordinator, Behavioral Health, you'll play a vital role in coordinating patient progress toward achieving quality and cost outcomes. Your expertise will help mobilize resources and manage the system in response to concurrent variances according to pre-established standards.
Key Responsibilities
• Utilization management (50%): Collaborate with physicians and multidisciplinary teams to ensure appropriate management of resources to attain patients' individualized goals in an appropriate and timely manner and setting.
• Implement effective discharge plans (20%): Ensure seamless transitions for patients, taking into account their unique needs and circumstances.
• Communication and collaboration (30%): Foster strong relationships with healthcare teams to ensure patients receive comprehensive care.
Requirements
• BA/BS degree and Nursing school diploma required
• RN license - State of PA
• 5 years acute Medical/Surgical and/or specialty care nursing required
• Experience in utilization management and/or employment by a third party payor/review agency preferred
