Job Summary

We are seeking a highly skilled and compassionate Home Health Visit Nurse to join our team at DCH Health System. As a Home Health Visit Nurse, you will play a vital role in providing high-quality patient care and collaborating with our interdisciplinary team to achieve optimal patient outcomes.

Key Responsibilities
  • Provide skilled nursing care to patients in their homes, including assessments, treatments, and education on proper home health care strategies and procedures.
  • Collaborate with the Home Health RN Care Manager to develop and implement individualized plans of care that address patients' unique needs and goals.
  • Monitor and evaluate patient progress, making adjustments to the plan of care as needed to ensure optimal outcomes.
  • Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination and patient satisfaction.
  • Participate in performance improvement initiatives to enhance patient care and outcomes.
Requirements
  • Current Alabama RN license
  • 1 year of recent home health or acute care nursing experience or a combination of both
  • Extensive clinical knowledge and experience in caring for patients with multiple and complex diagnoses, disease processes, and care needs
  • Ability to lift 50 pounds, turn and position patients weighing over 200 pounds
  • Good communication skills and ability to work effectively in a team environment
Working Conditions

This position requires the ability to work in a variety of environments, including patients' homes, and to maintain a high level of professionalism and discretion when interacting with patients and families.
