Job Description

We are seeking a highly skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Aveanna Healthcare in Reading, PA. Responsibilities:
  • Conduct thorough patient assessments and collaborate with the interdisciplinary team to develop individualized care plans under the supervision of a Registered Nurse (RN).
  • Administer medications, treatments, and therapies as prescribed by the physician, ensuring accuracy and attention to detail.
  • Monitor patient progress and report any changes in condition to the supervising RN or physician, providing timely updates and recommendations for further care.
  • Provide education and guidance to patients and their families on healthcare management, medication administration, and self-care techniques, promoting empowerment and independence.
  • Collaborate with the healthcare team to ensure coordinated and comprehensive care delivery, addressing the unique needs of each patient.
  • Maintain accurate and up-to-date patient documentation in accordance with regulatory standards and organizational policies, ensuring compliance and quality assurance.
  • Offer emotional support and advocacy for patients and their families throughout the care continuum, fostering trust and confidence in our services.
Qualifications:
  • A current and unrestricted LPN license in the state of Pennsylvania, reflecting your commitment to excellence and patient care.
  • A minimum of six months of nursing experience, preferably in a home healthcare or pediatric setting, demonstrating your adaptability and expertise.
  • Strong clinical assessment and critical thinking skills, enabling you to identify potential issues and develop effective solutions.
  • Excellent communication and interpersonal skills, facilitating seamless collaboration with patients, families, and healthcare professionals.
  • Ability to work independently and collaboratively within a multidisciplinary team, prioritizing tasks and managing multiple responsibilities.
  • Reliable transportation and willingness to travel to patient homes within the Gilbertsville area, ensuring convenient and efficient care delivery.
