Job Summary

We are seeking a highly motivated and compassionate Community Health Worker to join our team at UnitedHealth Group. As a Community Health Worker, you will play a vital role in assessing, planning, and implementing care strategies that are individualized by patient and directed toward the most appropriate, least restrictive level of care.

Key Responsibilities
  • Assess patient needs and develop personalized care plans
  • Collaborate with patients, families, and healthcare providers to ensure seamless care coordination
  • Identify and initiate referrals for social service programs, including financial, psychosocial, community, and state supportive services
  • Provide education and support to patients and families to promote healthy behaviors and self-management
  • Document all patient interactions and care plans in a timely and accurate manner
Requirements
  • 4+ years of case management, community outreach work, or volunteering experience
  • 2+ years of experience working or volunteering within the Community Health setting in a field-based healthcare role
  • Intermediate level of computer proficiency, including Microsoft Outlook, Teams, and multiple web applications
  • Ability to work independently and maintain good judgment and accountability
  • Strong communication and customer service skills, both in person and via phone
