About the Role

At Ohio State University Wexner Medical Center, we are seeking a highly skilled Post Acute Relationship Consultant to join our Home-Based and Post-Acute Care Unit. This is an exciting opportunity to develop and implement programs and services that further our home care and post-acute care strategies.

The Job Description

This position will play a key role in implementing existing service offerings and supporting established/emerging relationships within the unit. The consultant will serve as a liaison between OSUWMC and external partners, working closely with Administrators, Physician Leadership, Case Management Leaders, Nursing and others to successfully operationalize programs.

Key Responsibilities

  • Develop and implement programs and services to support home-based and post-acute care efforts
  • Serve as a liaison between OSUWMC and external partners to support established/emerging relationships
  • Work closely with cross-functional teams to ensure quality, patient experience, operational, and financial performance targets are met
  • Collaborate with colleagues from across OSUWMC to provide planning, implementation, and consultation services
  • Support process improvement initiatives to enhance communication, key processes, policies, procedures, and protocols

Requirements and Qualifications

  • Masters degree in health or business administration or related field preferred, Bachelors degree required
  • Minimum of five years experience in a health system setting, preferably leading home care and/or post-acute care efforts
  • RN or other clinical background preferred
  • Knowledge of home and post-acute care network development practices, process improvement, and cost management initiatives
  • Effective interpersonal, verbal, and written communication skills essential

Benefits and Compensation

  • An estimated salary range for this position is $120,000 - $150,000 per year, depending on experience and qualifications
