We are seeking a compassionate and skilled Nursing Assistant to join our Hospice Care team at Advocate Health. As a Nursing Assistant, you will play a vital role in providing high-quality patient care and support to individuals and their families during a challenging time.
Responsibilities- Participate in the Admission/Discharge Transfer process of patients under the guidance of a GIP Hospice Nurse or Clinical Supervisor.
- Set up patient rooms, orient patients and families to the room, and obtain vital signs, including height and weight.
- Ensure all patient belongings are accounted for and documented during admission/transfer/discharge.
- Provide general patient care duties, including personal care, toileting needs, and skin care, under the direction of a GIP Hospice Nurse or Clinical Supervisor.
- Assist patients with activity orders, including proper positioning, ambulation, ROM, and special transferring needs.
- Respond to call lights and patient requests or refer to appropriate personnel.
- Collect, prepare, and transport patient specimens as directed by a GIP Hospice Nurse or Clinical Supervisor.
- Assist physicians with bedside procedures as deemed appropriate.
- Perform oral suctioning using a yankauer suctioning instrument, as delegated by an RN.
- Contribute to the patient plan of care through documentation and/or communication of patient needs, problems, and outcomes to other healthcare providers.
- High School diploma or equivalent.
- Completion of CAN program preferred.
- Certified Nursing Assistant certification required.
- CPR certification required.
- Ability to work various shifts, including holidays, to provide coverage.
- Ability to lift 50lbs or more may be required.
This position requires the ability to work in a fast-paced environment, with exposure to various contagious diseases, radiation, chemicals, caustic materials, electrical equipment, and hazards associated with their use.
Ability to lift up to 35 pounds without assistance. For patient lifts of over 35 pounds, or when the patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available.
