Job Overview

Role Responsibilities
  • As an Occupational Therapist, you will act as a patient's partner throughout their plan of care. This involves ensuring the restoration of motion to leave each client functioning at their personal best and safe in their home.
  • You will work to maximize quality of life and movement potential within areas of promotion, prevention, diagnosis, treatment or intervention, and rehabilitation.
What You Would be Responsible For:
  • Implement established policies and procedures.
  • Improve or minimize residual physical disabilities of patients.
  • Return individuals to their optimum and productive level within their capabilities.
  • Participate with other healthcare personnel in patient care planning.
  • Takes initial history and makes initial evaluation.
  • Perform all skilled procedures as ordered by physicians.
  • Consults with physicians regarding changes in treatment.
  • Instructs patients and family/significant others in home programs and activities of daily living.
  • Attends all patient care conferences as scheduled.
  • Prepares clinical and progress notes.
  • Helps develop the plan of care and revise as necessary.
  • Consults with family and Agency personnel.
  • Completes and submits OASIS assessments, reassessments, and transfers, resumptions of care, discharges and significant change in condition in accordance with Agency defined time frames.
Qualification Standards
  • Current License in state of residence.
  • Two (2) years' Home Health experience, preferred.
  • Acceptance of philosophy and goals of this Agency.
  • Ability to exercise initiative and independent judgment.
  • Intensive education and clinical expertise.
  • Ability to manage patients with different types of personalities.
  • Current knowledge of treatment practices.
  • Drive to continually learn and grow.
  • Professional behavior.
  • Excellent interpersonal communication skills.
About Compensation

The estimated annual salary for this role is $75,000-$90,000 based on industry standards and location considerations. This figure may vary depending on factors such as experience and qualifications.
