About the Role

We are seeking a skilled and compassionate Home Health Physical Therapist to join our team at HCA Florida Healthcare at Home. As a key member of our care delivery team, you will provide high-quality, patient-centered care by managing the complete process of therapeutic care for assigned patients.

This is a 32-hour per week position, offering a fulfilling opportunity for a dedicated professional who shares our commitment to delivering exceptional healthcare services. Key Responsibilities
  • Provide evaluations and treatment interventions within the scope of practice established by law and professional code of ethics.
  • Assess and evaluate therapeutic/rehabilitative/functional status and participate in the development of the total plan of care.
  • Deliver purposeful and skilled therapeutic care at patients' homes, utilizing procedures and techniques to produce a change in condition/function consistent with the physician's plan of care.
  • Instruct patients/family/caregivers in the use of therapies, exercises, therapeutic appliances, and/or prosthetic devices to be used outside of therapy sessions.
  • Prioritize caseloads, determine daily staffing needs, and coordinate assignments/handoffs.

Qualifications and Requirements
  • Bachelor of Science in Physical Therapy from an accredited program.
  • Certifications, Licenses, and other Special Requirements.
  • Completion of the National Certification Examination for Registered Physical Therapist.
