We are seeking a highly skilled Registered Nurse to fill the role of Patient Care Manager at Adoration Home Health and Hospice. As a key member of our team, this individual will be responsible for overseeing patient care and ensuring that our patients receive the highest quality support.

The ideal candidate will have prior supervisory experience and a proven track record of driving results. They should also possess knowledge of home health and Medicare home healthcare regulations.

Responsibilities:

Care Management

This RN/Patient Care Manager will oversee the preparation and submission of Requests for Anticipated Payment (RAPs) and final claims for billing according to regulatory requirements.

In addition, they will hold weekly patient care team conferences, in-service training, and staff meetings. The successful candidate will also be responsible for interviewing, hiring, and providing new hire orientation and performance evaluations.

Market Development

This role involves participating in community outreach initiatives by building relationships with physicians, hospitals, and nursing homes to increase patient referrals and grow market share within designated service areas.

To succeed in this position, you must have:

  • A Registered Nurse (RN) license in good standing in the applicable state
  • Prior supervisory experience and a proven track record of driving results
  • Demonstrated knowledge of home health and Medicare home healthcare regulations
  • Familiarity with medical software and equipment (OASIS and WellSky preferred)
  • Ability to build rapport with patients
  • Strong problem-solving and critical thinking skills
  • Reliable transportation, valid driver's license, and current automobile insurance
