The Opportunity

We are seeking a Healthcare Support Specialist to join our primary care team in Phoenix, AZ. As a key member of our team, you will play a crucial role in supporting patients with preventive, chronic, and acute care needs. Your strong communication skills and empathy will enable you to establish trusting relationships with patients and their families.

Responsibilities:
  • Capture relevant patient information during rooming processes, take vitals, administer point-of-care testing, and perform standard screening assessments.
  • Assist with post-visit care coordination by scheduling appointments, coordinating referrals, and sharing information with internal and external care teams.
  • Collaborate with providers to monitor patient health and ensure they receive necessary preventive measures.
  • Participate in daily practice operations, including answering phone calls, responding to emails, and maintaining a clean clinical environment.
  • Provide patient education on chronic disease management and coach patients using motivational interviewing techniques.
Requirements:
  • Graduation from an accredited postsecondary program or completion of relevant Medical Assistant training.
  • Strong written and verbal communication skills.
  • Experience working on diverse, feedback-driven teams.
  • A proven track record of persisting through change and demonstrating a forward-thinking perspective.
  • Proficiency in computer technology, including typing, internet navigation, and multiple software systems.
