About the Role

We are seeking a dedicated Licensed Practical Nurse (LPN) to join our team at Saber Healthcare Group. As a LPN, you will play a vital role in providing high-quality care to our residents in a long-term care setting.

Responsibilities
  • Provide direct patient care, including administering medications, wound care, and vital sign monitoring.
  • Develop and implement individualized care plans to meet the unique needs of each resident.
  • Collaborate with the interdisciplinary team to ensure seamless care transitions and effective communication.
  • Participate in quality improvement initiatives to enhance the overall quality of care.
  • Document resident care and progress in a timely and accurate manner.
Requirements
  • Current, unencumbered LPN license in the state of employment.
  • Current CPR certification.
  • Ability to communicate effectively in English and follow verbal and written instructions.
  • Good interpersonal skills and attention to detail.
  • Ability to work with supervisors, co-workers, and community staff in the performance of duties.
