Job Title: Licensed Practical Nurse

We are seeking a skilled Licensed Practical Nurse to join our team at Northern Light Home Care and Hospice in Bangor. As a LPN, you will provide high-quality nursing care to patients in their homes, working collaboratively with our interdisciplinary team to deliver comprehensive care.

Responsibilities:
  • Provide direct patient care, including administering medications, performing wound care, and collecting specimens.
  • Conduct thorough assessments of patients' physical and emotional needs, developing and implementing individualized care plans.
  • Collaborate with registered nurses, therapists, and other healthcare professionals to ensure seamless care transitions.
  • Communicate effectively with patients, families, and caregivers to ensure understanding of care plans and progress.
  • Participate in ongoing education and training to stay current with best practices and regulatory requirements.
Requirements:
  • Current licensure as a Licensed Practical Nurse in the state of Maine.
  • High school diploma or equivalent required.
  • Ability to lift, move, and load 30-50 pounds, as well as prolonged periods of standing and walking.
  • Valid driver's license and reliable transportation.
