Job Summary:

As a Care Coordinator at LifeSkills, you will play a vital role in improving outcomes and patient satisfaction by providing direct, written, and telephonic contacts with clients and their service providers.

Key Responsibilities:

  • Provide case management and care coordination services to clients with substance use disorders
  • Develop and implement individualized care plans to ensure access to care and needed supports
  • Collaborate with clients, service providers, and other stakeholders to achieve optimal outcomes

Requirements:

  • BA degree in human services, psychology, social work, or sociology
  • Valid driver's license and insurable driving record
  • Previous experience in case management or care coordination preferred
