We are seeking a skilled Licensed Practical Nurse to join our team at Child & Family Services of NY. As a key member of our outpatient mental health clinic, you will play a vital role in providing education and support to youth, adults, and guardians regarding mental health services, systems, and medications.
Key Responsibilities- Collect vital signs, weights, and review medication benefits and side effects with clients receiving services from our psychiatric provider.
- Assure informed consents for medications and provide necessary information for proper dispensing at home and school.
- Communicate medication, behavioral, or primary health concerns to psychiatric providers, guardians, and clients' therapists.
- Document concerns in clients' charts and monitor as needed.
- Complete prior authorizations with insurance companies to ensure clients receive appropriate medications.
- Return phone calls from pharmacies to clarify script, refill, and other inquiries.
- Review laboratory test results and discuss concerns with psychiatric providers.
- Work collaboratively with mental health therapists regarding behavioral health concerns and outcomes.
- Ensure completion of required documentation in clients' charts.
- Spanish speaking preferred.
- LPN from a recognized accredited school of nursing.
- Two years of nursing experience preferred.
- Ability to relate well to children and other staff members.
