Job Overview

We are seeking a skilled Healthcare Provider to conduct comprehensive health assessments in patients' homes, providing critical insights into their overall well-being and enabling timely interventions.

About the Role

This is a PRN/Part Time position with flexible scheduling, allowing you to balance your work and personal life while making a meaningful impact on the healthcare outcomes of our patients.

Key Responsibilities
  • Conduct thorough adult and geriatric assessments, medical histories, diagnoses, treatments, health education, physician referrals, case management referrals, follow-up, and documentation according to Matrix guidelines and protocols.
  • Collaborate with physicians, case managers, social workers, family members, caregivers, and other relevant medical personnel as necessary.
  • Work closely with Primary Care Physicians (PCPs) to provide patient education and follow-up services.
  • Provide services in various settings, including home visits, skilled facility visits, and telehealth consultations.
Requirements and Qualifications
  • A Master's degree in a relevant field or equivalent experience, along with satisfactory completion of NP licensure.
  • Current RN and NP or PA licensure in the state of practice, including prescription authority or the ability to obtain it.
  • Board certification by a reputable organization, such as the NCCPA, AANP, ANCC, or AACCN, in a Matrix-approved specialty.
  • A valid driver's license and adequate automobile insurance coverage.
  • Strong computer skills and familiarity with EMR software.
  • Excellent verbal and written communication skills, with the ability to effectively interact with patients, clients, and colleagues.
  • A positive attitude, flexibility, and adaptability in a fast-paced environment.
Compensation and Benefits

This position offers a competitive compensation package, with an estimated salary of $85,000 per year, based on national averages and location. Additionally, you will have access to benefits, including a 401(k) plan and mileage reimbursement.
