We are seeking a highly skilled Licensed Clinical Social Worker to join our team at Drift Services. As a key member of our healthcare team, you will play a vital role in improving health care outcomes, supporting clients in managing their medications, and addressing barriers to care and treatment.
Key Responsibilities- Provide brief interventions focused on long-term achievable goals, working with clients to achieve viral suppression, better health outcomes, and quality of life.
- Build rapport, provide supportive listening, and ensure coordination of services to meet the unique needs of People Living with HIV (PLWH).
- Work with providers and the healthcare center team to ensure every Ryan White patient receives the support they require and access to available resources.
- Complete Comprehensive Assessments, develop Individualized Care Plans, and provide ongoing education to patients to ensure they understand their diagnoses and medications.
- Collaborate with the healthcare team to address mental health conditions or substance abuse patterns that interfere with a patient's ability to adhere to medical/medication regimens.
- LMSW or LCSW required.
- Current and good standing licensure as a Bachelor or Master's Social Worker in the state of Florida.
- Bilingual in Spanish preferred.
- 1 Year minimum of Case Management experience preferred.
- Knowledge of Ryan White Services preferred.
