A bachelor's degree in a human services-related field and a minimum of one year of relevant experience in direct service provision are essential qualifications.
Key Responsibilities
- Conduct comprehensive assessments to determine individual needs and ensure appropriate service delivery.
- Facilitate intake interviews and annual reviews, gathering necessary information in accordance with established guidelines.
- Assist in the completion of eligibility processes for services, ensuring compliance with all procedural requirements.
- Link individuals to community resources when they are found ineligible for certain services.
- Advocate for clients and their families, ensuring their active participation in the service planning process.
- Review and document individual needs across various domains, including medical, educational, and vocational.
- Coordinate and complete annual planning processes tailored to individual needs.
- Maintain accurate records of services provided and ensure compliance with agency standards.
- Collaborate with provider agencies to resolve service coordination issues effectively.
- Monitor and report on the services provided to specific client groups as required.
- Participate in team meetings and training sessions to enhance service delivery.
- Identify and report any service gaps or issues to management.
- Perform additional duties as assigned by supervisory staff.
Qualifications
- A bachelor's degree in human services is required, along with QIDP eligibility as per regulatory standards.
- Strong organizational skills and the ability to manage multiple tasks effectively.
- Knowledge of the developmental disabilities service system and related eligibility criteria.
- Ability to collaborate with a diverse range of professionals and families.
- Self-motivated with the capability to work independently and as part of a team.
- Flexibility to adjust work hours as needed.
- Commitment to ethical practices and professional communication.
- Reliable transportation and a valid driver's license are necessary.
Physical Requirements
- Effective communication skills, both written and verbal.
- Ability to perform physical tasks such as sitting, bending, and carrying files.
- Capability to lift light objects as needed.
