Key Responsibilities

  1. Conduct comprehensive assessments to determine individual needs and ensure appropriate service delivery.
  2. Facilitate intake interviews and annual reviews, gathering necessary information in accordance with established guidelines.
  3. Assist in the completion of eligibility processes for services, ensuring compliance with all procedural requirements.
  4. Link individuals to community resources when they are found ineligible for certain services.
  5. Advocate for clients and their families, ensuring their active participation in the service planning process.
  6. Review and document individual needs across various domains, including medical, educational, and vocational.
  7. Coordinate and complete annual planning processes tailored to individual needs.
  8. Maintain accurate records of services provided and ensure compliance with agency standards.
  9. Collaborate with provider agencies to resolve service coordination issues effectively.
  10. Monitor and report on the services provided to specific client groups as required.
  11. Participate in team meetings and training sessions to enhance service delivery.
  12. Identify and report any service gaps or issues to management.
  13. Perform additional duties as assigned by supervisory staff.

Qualifications

  1. A bachelor's degree in human services is required, along with QIDP eligibility as per regulatory standards.
  2. Strong organizational skills and the ability to manage multiple tasks effectively.
  3. Knowledge of the developmental disabilities service system and related eligibility criteria.
  4. Ability to collaborate with a diverse range of professionals and families.
  5. Self-motivated with the capability to work independently and as part of a team.
  6. Flexibility to adjust work hours as needed.
  7. Commitment to ethical practices and professional communication.
  8. Reliable transportation and a valid driver's license are necessary.

Physical Requirements

  1. Effective communication skills, both written and verbal.
  2. Ability to perform physical tasks such as sitting, bending, and carrying files.
  3. Capability to lift light objects as needed.
