Job Summary

We are seeking a highly skilled and compassionate Social Services Professional to join our team at SSM Health. Key Responsibilities
  • Conduct thorough interviews with patients to assess their needs and priorities, documenting case activity accurately.
  • Collaborate with healthcare professionals and community resources to develop and implement effective care plans.
  • Assist patients and families in accessing community assistance programs, referrals, and resources.
  • Develop and maintain relationships with community health, welfare, and social agencies to ensure seamless support for patients.
  • Participate in quality improvement initiatives to enhance standard work processes.
Requirements
  • Master's degree in Social Work.
  • No prior experience required, but a strong passion for pediatric behavioral health and social services is essential.
