About the Role:

The Lead Clinical Social Worker provides supervisory support to the Care Management Department social workers and directs the Maternal Child Health (MCH) services social work programs in support of patient safety and compliance with Maryland Statute and Social Work practice guidelines.

Key Responsibilities:
  • Coordinate and direct discharge planning, social services, and care coordination throughout the hospital and with outside agencies.
  • Ensure social services, discharge planning, and care coordination needs of patients and their families are met in a patient-centered, effective, and efficient manner.
Requirements:
  • State of Maryland LGSW, LCSW, or LCSW-C licensure.
  • Masters Degree in Social Work.
  • Certified as a clinical supervisor in the state of Maryland preferred.
  • Three or more years hospital social work experience or protective services experience.
