Healthcare Provider Associate

We are seeking a skilled Healthcare Provider Associate to join our team at RWJBarnabas Health. In this role, you will be responsible for providing high-quality patient care and support under the direct supervision of a registered professional nurse.
About the Role:
This per diem opportunity requires a qualified individual to administer nursing care under the direct supervision of a registered professional nurse. Your responsibilities will include working with patients/family and other team members to provide age-appropriate health care needs, practicing within the guidelines of the State Nurse Practice Act. You will also be responsible for coordinating patient follow-up of appointments, tests, and plan as per the provider's instructions, and documenting patient care and follow-up in the medical record.
Key Responsibilities:
* Work collaboratively with patients, family, and healthcare team members to provide patient-centered care.
* Develop and implement individualized care plans that meet the unique needs of each patient.
* Coordinate patient care activities, including scheduling appointments and communicating with healthcare providers.
* Maintain accurate and up-to-date patient records, including documentation of medication administration and patient interactions.
* Participate in quality improvement initiatives to improve patient outcomes and safety.
