Occupational Therapist Job Description

Green Hills Center for Rehabilitation and Healing is seeking a skilled Occupational Therapist to provide high-quality rehabilitation services to our patients.

Responsibilities:
  • Conduct thorough assessments of patients to identify their medical condition, functional capabilities, limitations, and potential for rehabilitation.
  • Develop and administer personalized treatment programs with specific goals tailored to each patient's capacity and tolerance under the direction of a physician.
  • Establish realistic and achievable goals for patients, document and record their condition, and educate patients and families on appropriate therapy methods.
  • Oversee and supervise Occupational Therapy Assistants.
  • Collaborate with all disciplines to provide patient-centered treatment and care.
  • Ensure exceptional patient service skills and demonstrate clinical excellence.
  • Adhere to scheduled hours, report to work on time, and demonstrate professionalism at all times.
  • Perform other duties assigned by the Director of Rehabilitation.
  • Comply with confidentiality, HIPAA, and Federal Residents' Rights.
Qualifications:
  • Graduation from an accredited Occupational Therapist college or university.
  • Valid professional license and registration for the state of practice.
  • Experience with long-term care and/or geriatric care is preferred but not limited to.
  • Well-versed in documentation writing, including evaluations, weekly progress notes, re-certification, and discharge documentation.
