As a Home Health Occupational Therapist with CenterWell Home Health, you will play a vital role in helping patients regain their independence and live healthier lives. Our care teams are dedicated to providing personalized, comprehensive home care for patients managing a chronic condition or recovering from injury, illness, surgery, or hospitalization.
Key Responsibilities- Assess and screen patients' daily living and work-related skills, developing therapeutic retraining programs with measurable objectives.
- Administer and interpret diagnostic and prognostic tests to assist physicians in evaluating patients' levels of function.
- Confer with physicians and other healthcare team members to develop and revise treatment plans.
- Provide therapeutic treatment and instruction to patients in accordance with physician orders, improving or restoring strength, coordination, range-of-motion, and function.
- Review and expand retraining programs to align with patients' behavioral goals and the overall rehabilitation program.
- Consult with vocational team members to develop and implement vocationally oriented plans.
- Report patient status and progress to physicians, rehabilitation staff, Clinical Managers, patients, and family members.
- Participate in care coordination and discharge planning activities, serving as a resource to other healthcare personnel.
- Design community reintegration activities to assist clients in physical reconditioning and psychological adjustment.
- Recommend and design special adaptive equipment to improve residence or working environments.
- Train staff and family members to carry out retraining programs, including dressing, feeding, grooming, and hygiene skills.
- Coordinate and oversee Occupational Therapy Assistant and Home Health Aide performance in implementing occupational therapy services.
- Accurately document patients' care observations, interventions, and evaluations.
- Degree from an accredited Occupational Therapy Program.
- A minimum of six months of occupational therapy experience preferred.
- Home Health experience a plus.
- Current and unrestricted OT licensure.
- Current CPR certification.
- Good organizational and communication skills.
- Valid driver's license, auto insurance, and reliable transportation.
