Job Summary:

We are seeking a highly skilled Community Health Consultant III, Social Health to join our team at Kaiser Permanente. This role will be responsible for independently managing key social health initiatives, programs, or services to meet KP's objectives and satisfy member social health needs. The successful candidate will design programs and initiatives, establish goals, engage in human-centered design, and utilize comprehensive knowledge to provide input into the design of enterprise engagement strategies and communication/change management plans.

Key Responsibilities:
  • Pursue effective relationships with others by proactively providing resources, information, advice, and expertise to coworkers and members. Listen to, seek, and address performance feedback; provide mentoring to team members. Pursue self-development; create plans and take action to capitalize on strengths and develop weaknesses; influence others through technical explanations and examples.
  • Complete work assignments autonomously by applying up-to-date expertise in the subject area to generate creative solutions; ensure all procedures and policies are followed; leverage an understanding of data and resources to support projects or initiatives. Collaborate cross-functionally to solve business problems; escalate issues or risks as appropriate; communicate progress and information.
  • Implement communication strategies by initiating communications between KP and management in community agencies regarding health programs; promoting KP's position and commitment to community health to internal and external audiences; participating in corporate marketing, community relations outreach, and identifying media opportunities to increase visibility of community health portfolio and programs.
  • Execute community health program management by completing basic work for community health programs in KP's Community Health Program, with limited guidance; ensuring that all community health programs and activities meet the needs of KP's diverse communities; implementing community health strategies and programs across the market, with guidance, including philanthropy, in-kind, and/or community health improvement programs at the local and/or market level.
  • Ensure regulatory compliance by ensuring that activities are in compliance with legislative, regulatory, and KP policies; and collecting information to support all regulatory, governmental, and KP reporting requirements, independently.
  • Leverage organizational resources by utilizing comprehensive foundational knowledge in leveraging in-kind resources from KP, including technical expertise, programs managed by KP, and operational assets to connect to, support, and improve the health of KP communities and organizations.
  • Build stakeholder relationships by managing relationships with stakeholders including non-profits, for-profits, elected officials, public agencies, and other internal stakeholders to engage in community health work; engaging market and/or program office leadership and other key internal stakeholders to support execution of community health strategies.
  • Execute strategic planning by supporting the development of community health strategic plans; planning and implementing national and market community health programs, projects, and initiatives; supporting the strategic development of a market-wide integrated community health plan which supports the organization's strategic imperatives and ensures alignment to local medical service area needs.
Requirements:
  • Bachelor's degree from an accredited college or university AND minimum three (3) years of experience in government, public health, health services, economic development, community/social/human services, or a related field OR minimum six (6) years of experience in government, public health, health services, economic development, community/social/human services, or a related field.
Preferred Qualifications:
  • One (1) year of experience in project management.
  • Valid driver's license in location where applicable.
