The Registered Nurse (RN) will operate under the guidance of the Nurse Manager/Clinical Coordinator. This role involves assessing, planning, implementing, evaluating, and supervising individualized patient care within a nursing unit or department, adhering to established policies and procedures. The RN will also oversee non-RN personnel on the assigned unit and, when in a charge role, will delegate and supervise unit activities. Flexibility to work across different nursing units may be required based on patient and staff distribution. Additional responsibilities may be assigned to ensure a high standard of service delivery.
Key Responsibilities
- 1. Continuously assess the physical and psychosocial condition of patients throughout their care journey.
- 2. Develop, communicate, and document a personalized care plan that addresses individual patient needs and interventions, engaging the patient, family, and healthcare team in the process.
- 3. Implement and coordinate the care plan in collaboration with the healthcare team.
- 4. Evaluate the effectiveness of the care plan, monitor patient responses, and readiness for discharge, revising the plan as necessary and making referrals when appropriate.
- 5. Plan educational initiatives based on the individual learning needs and readiness of patients and families, providing educational opportunities and documenting outcomes.
- 6. Serve as a preceptor for new staff members.
- 7. Assume charge nurse responsibilities as needed.
- 8. Meet performance expectations related to customer service, teamwork, resource utilization, and professional development as outlined in performance evaluations.
- 9. Perform additional duties as assigned to ensure the smooth operation of the department/unit.
- 10. Demonstrate skill and understanding in the care of patients receiving chemotherapy, in accordance with established policies and procedures.
- 11. Exhibit proficiency in accessing Port-A-Caths and initiating IV access.
- 12. Administer chemotherapy following established protocols.
- 13. Adhere to safety standards for chemotherapy administration.
- 14. Show competency in caring for hospice patients and their families.
Education
Graduation from an accredited nursing program; BSN preferred. Proficient in the use of computer software, including electronic patient records, and capable of conducting literature searches online for patient care information.
Experience
Experience in medical, surgical, oncology, and chemotherapy settings is preferred.
Licensure
Current Registered Nurse License in the State of Connecticut.
Special Skills
BLS certification is required.
