Job Title: Home Health Physical Therapist

VNA Health Group is seeking a skilled Physical Therapist to join our team in Edgewater, New Jersey.

Job Summary:

We are looking for a dedicated Physical Therapist to provide high-quality care to patients in their homes or alternate community settings. The ideal candidate will have a strong background in physical therapy, excellent communication skills, and a passion for delivering compassionate care.

Responsibilities:
  • Conduct thorough assessments and evaluations of patients to identify their physical therapy needs.
  • Develop and implement personalized plans of action in collaboration with physicians to ensure optimal patient outcomes.
  • Provide skilled physical therapy interventions to patients in their homes or alternate community settings.
  • Monitor patient progress, update care plans, and document all changes in patient condition.
  • Identify counseling needs and develop effective teaching strategies to support patient education.
  • Adhere to agency infection control and safety policies, as well as organizational policies regarding absenteeism and appearance.
Requirements:
  • Physical Therapy Licensure in New Jersey is required.
  • 2-3 years of prior clinical experience and community-based experience are preferred.
  • Strong computer skills and excellent communication and interpersonal skills are essential.
  • A valid New Jersey driver's license is required.
