Clinical Pharmacist

We are seeking a highly motivated and qualified Clinical Pharmacist to join our team at the Clallam Bay Corrections Center. As a Clinical Pharmacist, you will play a critical role in enhancing patient health and wellness through evidence-based pharmaceutical care.
Key Responsibilities- Provide clinical pharmacy services to patients in a correctional setting, including medication therapy management and patient education.
- Collaborate with healthcare providers to individualize drug therapies and promote safe, effective, and economical patient care.
- Conduct patient assessments, monitoring, and reviews of patient health records to identify and mitigate drug-related problems.
- Make evidence-based medication recommendations and provide comprehensive written and/or oral medication information to healthcare professionals.
- Participate in clinic/infirmary rounds and/or conferences with other healthcare professionals to assess medication therapy and recommend management solutions.
- Two or more years of clinical consultative pharmacy experience.
- A valid Washington State Pharmacist License.
- Desired qualifications include pharmacy residency, Board of Pharmacy Specialties certification, and hospital, long-term care, or managed care pharmacy experience.
