Job Summary:

The Clinical Pharmacist at Bay Area Hospital is responsible for providing comprehensive pharmaceutical care services to patients. This includes assessing medication therapy, providing clinical interventions, and collaborating with multidisciplinary care providers to ensure safe and effective medication use.

Responsibilities:

  • Assess medication therapy and provide clinical interventions to ensure safe and effective medication use
  • Collaborate with multidisciplinary care providers to monitor and assess patient care needs
  • Provide support for centralized and decentralized services
  • Deliver optimal medication therapy to patients with a broad range of disease states

Requirements:

  • Graduation from an accredited College of Pharmacy with Doctor of Pharmacy (Ph.D.) degree preferred or B.S. Pharm degree
  • Current Oregon Pharmacist license required
  • American Heart Association Basic Life Support (BLS) certification
  • American Heart Association Advanced Cardiovascular Life Support (ACLS) Certification required (must be obtained within 6 months of hire)
  • A minimum of 3 years of relevant hospital experience preferred
