Hospice Care Specialist

We are currently seeking skilled Licensed Practical Nurses (LPNs) to join our PruittHealth Hospice team in North Carolina. This is a rewarding opportunity to provide palliative care and ongoing support to patients and their families in the comfort of their own homes.
About the RoleThis hospice LPN position offers a collaborative team environment, competitive compensation, and flexible schedules. As a member of our interdisciplinary team, you will work closely with patients, caregivers, physicians, and other healthcare professionals to deliver comprehensive care.
- Provide palliative nursing care and ongoing support in the home setting.
- Function as a team member, demonstrating collaboration and responsibility to patient, caregiver, physician, and interdisciplinary team.
- Carry out pain management and symptom management plans.
- Respond to physical, mental, social, and emotional needs of terminally ill patients and their families.
- Adhere to agency, payroll, billing, and documentation policies and procedures.
- Palliative care: Providing compassionate nursing care to patients in their homes, focusing on alleviating symptoms and improving quality of life.
- Pain management: Collaborating with the interdisciplinary team to develop and implement individualized pain management plans.
- Symptom management: Monitoring and addressing physical, mental, and emotional symptoms associated with terminal illnesses.
- Care coordination: Working closely with patients, caregivers, and healthcare providers to ensure seamless transitions and coordinated care.
- Disease education: Educating patients and families about disease management, treatment options, and available resources.
- Current, active, and unrestricted Licensed Practical Nurse (LPN) licensure in the state of practice.
