RN Triage Nurse

We are seeking a skilled and compassionate Registered Nurse to join our team as a Triage Nurse in Jacksonville, FL. As a Triage Nurse, you will play a critical role in assessing patients' health problems and needs, developing and executing nursing care plans, and assisting in the maintenance of patient medical records.
Key Responsibilities- Assess patients' health problems and needs, and develop and execute nursing care plans.
- Collaborate with the care team and clinicians to provide high-quality patient care.
- Identify and address patient care needs by establishing personal rapport with patients and employing a variety of strategies, approaches, and techniques to manage a member's physical, environmental, and psycho-social health issues.
- Work in partnership with the center's clinicians, front office staff, care integration team, and other team members to anticipate and resolve patient needs.
- Help patients identify and resolve barriers that hinder effective care.
- Understand organizational, market, and center-level operating goals and metrics.
- Licensed Registered Nurse with no disciplinary action.
- Ability to work independently under general instructions with a team with good communication and interpersonal skills.
- Ability to work with minimal supervision within the role and scope.
- Prior clinical experience in ER, Hospital, and/or ambulatory patient education.
- Prior Call center or Triage experience.
- Prior clinical experience in case management.
- Intermediate knowledge of Microsoft Word, Outlook, Excel, and prior EMR exposure.
- Bilingual is a plus.
