We are seeking a highly skilled and compassionate LPN to join our team as a Patient Care Advocate in our Ambulatory Clinic. As a Patient Care Advocate, you will play a critical role in ensuring the seamless coordination of patient care, from initial outreach to ongoing support and follow-up.
Key Responsibilities- Outreach and Coordination: Develop and implement effective outreach strategies to identify patients who require care for various medical conditions. Coordinate with patients, primary care providers, and staff to ensure timely and efficient care.
- Quality Improvement: Collaborate with the Physician Enterprise team to design, implement, and monitor quality improvement initiatives. Conduct data analysis, develop project plans, and provide education to team members and providers on quality measures and processes.
- Clinical Competency: Develop and implement training plans to educate end-users on assigned projects. Facilitate end-user training programs and ensure proper training materials are available and provided prior to and during training.
- Patient Engagement: Engage with patients and their families to ensure they understand their care plans and are empowered to take an active role in their health. Communicate effectively and courteously with patients and colleagues, incorporating excellent verbal, written, and listening communication skills.
- Administrative Support: Provide administrative support to the team, including checking on patients, ensuring they attend appointments and follow-ups, and maintaining accurate records.
- Education and Experience: Three years of clinical experience required, with three or more years of EMR experience preferred. LPN licensure in the state of Georgia is mandatory.
- Skills and Qualities: Strong attention to detail, personal commitment to accuracy and excellence, excellent communication and interpersonal skills, and demonstrated organizational skills.
