Job Summary

We are seeking a highly skilled and compassionate Case Manager to join our Children and Youth Services team at Service Access & Management, Inc. As a key member of our team, you will be responsible for providing case management support to families and children in need.

Key Responsibilities
  • Evaluate and Assess the safety and well-being of referred children, working closely with families to identify strengths and areas for improvement.
  • Develop and Implement tailored service plans to meet the unique needs of each family, ensuring the overall welfare and best interests of the child.
  • Collaborate and Coordinate with various systems, including legal and law enforcement entities, to safeguard children and ensure comprehensive care.
  • Provide Ongoing Support to families, including regular check-ins, home visits, and community-based services.
  • Document and Record all services provided, ensuring accurate and timely documentation.
Requirements
  • Bachelor's degree with 12 college credits in a relevant field, or equivalent experience with 12 college credits and one year in a similar role.
  • Valid driver's license and insurance, clearances, fingerprinting, drug screening, and other necessary certifications.
  • Ability to lift up to 15 pounds and work in a primarily office-based environment with some travel within the region.
