In this role, you will have the opportunity to develop and implement personalized care plans that cater to the unique needs and preferences of each program participant.

Responsibilities:

  • Provide person-centered care and support to program participants, enabling them to reach their full potential.
  • Assist with daily living tasks, such as bathing, dressing, and toileting.
  • Monitor and record progress on daily checklists, behavior contracts, quarterly summaries, and monthly goal forms.
  • Communicate effectively with program participants, families, and healthcare professionals.

Requirements:

A High School diploma or equivalency diploma is required. Experience working with adults with intellectual or developmental disabilities is preferred.Certification in CPR, First Aid, AMAP, and SCIP is mandatory.Strong communication and interpersonal skills are essential for this role.
